Surgery has been standard treatment for acute appendicitis for over a century. Appendectomy is the most common emergency abdominal surgery as acute appendicitis has been thought to always progress to perforation. We now know that complicated severe and uncomplicated mild appendicitis are two different forms; the majority of cases are mild. Recently promising results in the treatment of uncomplicated appendicitis with antibiotics instead of surgery have been published by the study group in the currently largest trial. Time has come to abandon routine surgery for mild appendicitis. Avoiding unnecessary surgeries will result in major cost savings and decreased morbidity for patients. Study project aims to evaluate optimization of antibiotic treatment for uncomplicated appendicitis and etiology of the different diseases by microbiology and immunology. Patients with CT scan confirmed uncomplicated appendicitis will be enrolled in multicenter treatment RCTs optimizing antibiotic therapy.
